12 Individual monthly spreadsheets, one for each month. Enter Date, Description, Odometer Start, Odometer Finish, Rate paid per mile and your miles and amount owed to you are totaled your for mileage.
Summary Page
The thirteenth and final page is a summary which tracks your total miles and total amount owed to you for the whole year.
