Bookends is a very simple but powerful MacosX bibliographic reference manager. Use it to download references from Pubmed easily. Then export them as tab-delimited. Use Handbase desktop 3.0i for Mac to convert that file to a handbase file and voila... all your medline references appear on your palm.
