Personal Health Record for HanDBase - This is a suite of 15 databases that can be used to keep track of your personal health information as well as other members of your family. For each separate person kept track of in this suite, the following information can be recorded: identification, emergency contact, allergies, dental visits, infectious diseases, growth and development, health problems, healthcare providers, hospitalizations, immunizations, labs or imaging, medications, surgeries, and vision information.
These databases will be installed under a "Health" category in your HanDBase application. Please read the README.txt file in the health_record.zip file for more comments.
